LIFE- CYCLE OF A FROG

Purpose Learning Outcome
Life Cycle of a Frog
My assessment plan and outcome is for pre-k. The sole purpose of my assessment plan and outcome is to ensure that the students will have gained knowledge by the end of this unit on "Frogs". Students will have learned the four stages of the life-cycle of a frog. They will be able to identify each stage and use vocabulary related to the stages of the life-cycle of a frog. Students will have gained the knowledge and understanding of the life-cycle of a frog. The assessment plan will be an evaluation through performance based on their testing. Students will exhibit proficiency in the content area by explaining, demonstrating, and supporting their skills through the assessment process and through project based assessment.
Outcomes:
1. Students will identify and place in sequence the four stages of development of a frog.
2. Students will use scientific vocabulary related to the life-cycle of a frog (metamorphosis, egg, tadpole, and adult frog).
3. Students will be able to describe the metamorphosis and what happens to a frog during the four stages of the life-cycle of a frog.
4. Students will present their organized work in small group and demonstrating what skills they have learned.
Assessment Context:
1.      Using a four square chart labeled 1 through 4, and four pictures with the stages of a frog, students will cut and glue them into sequence order on the chart. Only the sequence order will be assessed.
2.       During small group, using the book "Tale of a Tadpole” and props, students will use vocabulary (metamorphosis, egg, tadpole, and adult frog) by taking turns reading the book and describing with the life-cycle of a frog. Students will be good listeners and help fellow students who may forget the sequence of events.
3.      Students will draw a picture to demonstrate the metamorphosis by labeling/Identifying the changes of a tadpole to an adult frog during the four stages of its life-cycle in sequential order.
Testing Constraints:
1.      The students will work independently during the assessment, with the guidance being the directions given and review.  This activity should come easy for the students, since they would have done this as a previous activity in the learning process. The students would only need a reminder as to the ordering from first to last. Completion of this assessment would be a 15min.
2.      An observational assessment will be assessed with the teacher using a checklist as each student takes turn reading “Tale of a Tadpole”. Each child will be allowed 8/10 minutes to complete their turn depending on the child.
3.      Students will be allowed 20 minutes to complete their assessment. After completing their illustrations students will record their knowledge of the life-cycle of a frog including metamorphosis by using a tape recorder. Then each student will be given at least 5 minutes to present what they have learned to their classmates.
